This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

McFadden Institute, LLC ("the Practice," "we," "our") is required by the Health Insurance Portability and Accountability Act ("HIPAA") to maintain the privacy of your protected health information ("PHI"), to provide you with this Notice of our legal duties and privacy practices regarding your PHI, and to notify you following a breach of unsecured PHI.

Uses and disclosures of PHI

We may use and disclose your PHI without your written authorization for the following purposes:

• Treatment. To provide, coordinate, and manage your clinical care, including consultation with other treating providers where clinically appropriate.
• Payment. To bill for services, arrange payment, or provide a superbill you may submit to your insurer for out-of-network reimbursement.
• Healthcare operations. For quality assessment, staff review, licensure activities, business planning, and other practice-management purposes.
• Required by law. When disclosure is required by federal, state, or local law, including reports of suspected abuse, neglect, or domestic violence, and responses to court orders.
• Serious threat to health or safety. To prevent a serious and imminent threat to you or another person.

Any other use or disclosure of your PHI will only be made with your written authorization, which you may revoke at any time in writing.

Your rights

• The right to inspect and receive a copy of your PHI.
• The right to request an amendment to your PHI.
• The right to an accounting of certain disclosures of your PHI.
• The right to request restrictions on how we use or disclose your PHI.
• The right to request that communications be sent by alternative means or to alternative locations.
• The right to receive a paper copy of this Notice.
• The right to be notified following a breach of unsecured PHI.

Our legal duties

We are required by law to maintain the privacy of your PHI, to provide you with this Notice, and to abide by the terms of the Notice currently in effect. We reserve the right to change our privacy practices and the terms of this Notice at any time, and to make the new terms effective for all PHI we maintain.

Telehealth and secure platforms

All clinical sessions are delivered over HIPAA-compliant telehealth platforms. This public website does not collect protected health information. Any clinical information is gathered only within secure, HIPAA-compliant systems after you become a client.

Complaints
