# High-Functioning Depression

From the outside, you're fine. Better than fine. Inside, it's gray and quiet and very tired.

How RRT works: https://mcfaddeninstitute.com/approach

## What this looks like

You hit every deadline and feel nothing when you do.

People describe you as the reliable one.

Joy has been replaced by relief that the day is over.

Keeping it together takes everything you have.

## How Rapid Resolution Therapy approaches it

RRT looks at what the mind is holding, not at how well you're performing.

You don't have to fall apart to deserve help.

## Not a diagnosis, but a real pattern

High-functioning depression isn't a formal diagnostic term. It's a way people describe something very recognizable: you do everything a well person does, and you feel very little of what a well person feels.

It sits within the broader experience of depression (https://mcfaddeninstitute.com/specialties/depression), just hidden better.

## Performing well while flat inside

You show up, you deliver, you remember everyone's birthday. Then you get in the car and stare at the steering wheel for ten minutes before you can drive home. Weekends are for recovering enough to do it again. Nothing is wrong, exactly. Nothing is right, either.

## Why nobody notices

Because you're good at hiding it, often without meaning to. Competence reads as wellness. People who rely on you have no reason to look closer, and saying something might feel like letting them down.

It's especially common in people with demanding roles. If that's you, the professional burnout (https://mcfaddeninstitute.com/specialties/professional-burnout) page is worth reading; the two often overlap.

## The exhaustion of keeping it up

Holding it together is a job with no days off. The effort it takes to look fine is often what's draining you most. Many people have spent years in weekly talk therapy getting insight into why, and still feel the same. If that's familiar, you might find it useful to read how RRT differs from traditional talk therapy (https://mcfaddeninstitute.com/approach/rrt-vs-talk-therapy).

## The medical side

Please see a physician to rule out physical contributors like thyroid or sleep problems. If you want to explore medication, that conversation belongs with a physician or psychiatrist. I don't prescribe. Therapy can run alongside medical care.

In session, we look at what your mind is holding underneath the performance. No homework stack, no pep talks.

## What it can look like to let someone in

For people used to being the capable one, reaching out can feel strange, even embarrassing. You might downplay it in the first call. That's fine. You don't have to prove anything to me. The consultation is just a conversation about what's going on and whether this work fits.

In sessions, you won't need to perform. There's no grade, no homework to keep up with, no one to impress. That, for many people, is a relief in itself.

Sessions are 90 minutes by video, booked within weekday hours, so they can sit around a demanding schedule rather than on top of it.

## Why it's easy to keep putting this off

Depression that hides behind competence has a built-in excuse: nothing is falling apart, so it never feels urgent. There's always a deadline, a family event, a busy quarter. You tell yourself you'll deal with it when things calm down, and things don't calm down.

It's also easy to talk yourself out of it by comparison. Other people have it worse. You're still getting up, still showing up. Asking for help can feel like an overreaction, or like admitting something you've worked hard to keep private.

But the cost of waiting is paid quietly, in years that feel gray. You don't need a crisis to justify a conversation. A free 15-minute consultation takes less time than most meetings, and if you want a sense of what comes after, here's how a 90-minute session works (https://mcfaddeninstitute.com/approach/90-minute-sessions). Starting small is still starting. If work is at the center of it, burnout or depression? (https://mcfaddeninstitute.com/resources/burnout-vs-depression) may help you sort the two out.

If you're in crisis, call or text 988 (Suicide & Crisis Lifeline). For emergencies, dial 911.

Competence can be a very convincing disguise.

## Questions people ask

### Is high-functioning depression a real diagnosis?

It's not a formal diagnostic term. The experience it describes is real, though, and you don't need a formal label to get help with it.

### Do I need to be “bad enough” to get help?

No. There's no threshold you have to reach. If the way you feel is getting in the way of living, that's reason enough to talk.

### Do you prescribe medication?

No. I'm a licensed clinical social worker. Medication questions go to a physician or psychiatrist, and therapy can run alongside that.

Not sure if what you're carrying fits here?

A free 15-minute call is the simplest way to find out.
